Two-factor authentication issue

Resolved
From 9:12am until 10:10am Eastern Time (13:12 - 14:10 UTC), our two-factor authentication system failed to authenticate valid requests. If you were asked for a two-factor authentication between those times, your code may have been rejected or you may not have received the code via SMS.

This issue has been resolved and two-factor authentication is working normally.

We apologize for this issue and will be working to prevent this type of failure in the future.
